Application for Fire Systems Interruption

Should your project require the isolation of the fire services because of works that generate heat, dust, or smoke, then the following procedures should be strictly adhered to:

Have the University Delegated Project Manager raise:

  1. A Work Request using the CIS Service Desk making sure to include a valid University Account Code to pay for the isolation and a comprehensive description of what and where the works are.
  2. This “Application for Fire System Interruption” form to be downloaded in PDF format or in Microsoft word format.
    This form is broken up into several sections and the following sections should be fully completed. These sections are as follows:
  • From whom, their position, their email address, and the date of the request
  • All Systems to be restored at the end of each day OR Systems left isolated overnight.( Should this last option be selected then the CIS Essential Services Department shall be consulted to ensure adequate fire protection of the location of the works. A Safe Work Method Statement shall be submitted with this Application for Fire System Interruption” form.)
  • Building number and name.
  • Building level, and room number/s.
  • Date and Time of Isolation & Date and Time of De-isolation.
  • Site contact and a Phone number to contact them on.
  • Comprehensive description that includes the expected duration of and the location of the works being undertaken.
  • Details of the fire systems to be interrupted in the area.
  • Work Request number.

This Form shall be submitted to the CIS Essential Services Department for their approval and to allow them to notify the various parties requiring forewarning of any interruptions.

Please submit this form a minimum of 72 hours prior to commencement of the works.

WORKS SHALL NOT COMMENCE BEFORE APPROVAL IS GIVEN.
