Dust Work permit

A Dust works Permit to be downloaded in PDF format or in Microsoft word format should be used for dusty works. This is a self-managed document, it is valid only for a 24hr period, so for works longer than 24 hrs a new form should be filled out for each 24hrs or part there of. At the completion of the works, a copy of these forms should be forwarded to the CIS Essential Services Department for their records.
